A transfusion reaction workup is a standardized, systematic laboratory investigation initiated when a transfusion reaction is suspected. Its primary purposes are: 1) **Confirm or rule out an immune-mediated hemolytic reaction** (the most critical to identify). 2) **Identify the type and cause of the reaction** (e.g., febrile, allergic, septic, TRALI, TACO). 3) **Prevent further harm** by determining if additional transfusions from the same donor or of the same component are safe. Key steps include: a) Re-checking all records for clerical errors (patient ID, unit ID, ABO compatibility). b) Visual inspection of post-transfusion urine for hemoglobin. c) Repeating ABO and Rh typing on pre- and post-transfusion patient samples. d) Performing a Direct Antiglobulin Test (DAT) on a post-transfusion sample. e) Re-crossmatching the implicated donor unit with the pre-transfusion patient sample. f) Culturing the blood unit and patient samples if bacterial sepsis is suspected. g) Testing for other causes (e.g., IgA deficiency, HLA antibodies). The findings guide clinical management and prevent future occurrences.
