Cushing's syndrome caused by an adrenal tumor is referred to as adrenal Cushing's syndrome or ACTH-independent Cushing's syndrome. In this condition, a benign adrenal adenoma or, less commonly, an adrenal carcinoma autonomously produces excessive amounts of cortisol, independent of the pituitary's control via ACTH. The high cortisol levels suppress ACTH production from the pituitary via negative feedback, leading to low or undetectable blood ACTH levels. Symptoms are those of hypercortisolism: central obesity, moon face, buffalo hump, purple striae, hypertension, glucose intolerance, muscle weakness, and osteoporosis. This differs from Cushing's disease, which is caused by a pituitary ACTH-secreting tumor (high ACTH). Treatment for adrenal Cushing's is typically surgical removal of the adrenal tumor (adrenalectomy).
