Multinucleated cells with ground-glass chromatin and molding best describes the cytological features of herpes simplex virus (HSV) infection in urine. HSV-infected cells become enlarged and may fuse to form multinucleated giant cells. The nuclei appear homogenously grayish (ground-glass) due to viral replication, and they often mold against each other. Intranuclear eosinophilic inclusions (Cowdry type A) may be present. HSV can cause cystitis or urethritis, and these cytopathic changes are the same as those seen in HSV infections at other sites like the genital tract or oral mucosa.
