Calretinin immunostain in pleural fluid cytology is a mesothelial marker used to distinguish mesothelioma (positive) from adenocarcinoma (negative). Calretinin shows nuclear and cytoplasmic staining in mesothelial cells. It is part of a panel that typically includes other mesothelial markers (WT1, D2-40) and adenocarcinoma markers (BerEP4, CEA, MOC-31). Calretinin is highly sensitive for mesothelioma but not entirely specific (some adenocarcinomas can show weak staining). Interpretation should be in context of morphology and other markers. The stain can be performed on cell blocks or liquid-based cytology preparations. Accurate distinction is critical for prognosis and treatment.
