The very first and most critical step in investigating any ABO typing discrepancy is to verify that you are working with the correct specimen. This involves double-checking that the information on the sample tube (patient's name, date of birth, unique ID) matches the test request form and the laboratory information system. You must also confirm that the sample was collected and labeled correctly. A surprisingly high number of discrepancies are caused by pre-analytical errors such as mislabeled tubes, wrong blood in tube, or clerical errors in transcription. Repeating the ABO typing on the same tube without this verification could simply repeat an error originating from a misidentified patient. Only after confirming specimen identity should technical troubleshooting (repeating tests, using different methods, checking patient history) begin.
