The Immediate Spin (IS) Crossmatch is a rapid serological compatibility test where the **recipient's serum** is mixed with the **donor's red blood cells**, centrifuged immediately at room temperature, and examined for agglutination or hemolysis. Its primary purpose is to **detect ABO incompatibility** between the donor unit and the recipient, as ABO antibodies (IgM) cause rapid agglutination at room temperature. **When is it considered sufficient?** The IS crossmatch is considered sufficient **only when the recipient meets specific low-risk criteria**: 1) The patient has a **current negative antibody screen**. 2) The patient has **no history of clinically significant antibodies**. 3) The patient's **ABO and Rh typing have been confirmed** on current and/or historical samples. For such patients, the IS crossmatch provides a quick check for ABO errors. However, it does NOT detect clinically significant IgG antibodies (e.g., anti-D, -K, -Fya). Many institutions have replaced the IS crossmatch with an **electronic (computer) crossmatch** for low-risk patients, which is faster and eliminates technical errors.
