Poor genital hygiene is considered one of the most significant modifiable risk factors for penile squamous cell carcinoma. In uncircumcised men, poor hygiene leads to the accumulation of smegma—a mixture of shed skin cells, oils, and moisture—under the foreskin. This substance is irritating and can cause chronic inflammation (balanoposthitis). Chronic inflammation is a known promoter of cellular changes that can lead to cancer over time. Furthermore, poor hygiene is associated with higher rates of infection with high-risk Human Papillomavirus (HPV) strains, another major risk factor. Therefore, good genital hygiene, including regular retraction and cleaning of the foreskin, is a crucial preventive measure. Neonatal circumcision, which removes the foreskin where these issues originate, is also a highly effective preventive strategy, but for uncircumcised men, hygiene is the key modifiable factor.
