Cell blocks from urine specimens are prepared by concentrating cellular material through centrifugation, forming a pellet that is fixed in formalin, processed like tissue, embedded in paraffin, and sectioned. Methods include: 1) Simple centrifugation with plasma-thrombin clot formation, 2) Agar embedding, 3) Commercially available cell block systems (e.g., Cellient). Cell blocks allow multiple sections for special stains (mucin, iron), immunohistochemistry, and molecular testing. They preserve architectural patterns better than smears. Adequate cellularity is required, so they are best prepared from specimens with high cellular yield like bladder washings.
