Inverse fever pattern (also called reversed or atypical fever) is characterized by higher temperatures in the morning and lower in the evening, opposite to the typical diurnal variation. This pattern is classically associated with certain conditions: tuberculosis (especially military TB); typhoid fever; brucellosis; drug fever; and some cases of lymphoma or connective tissue diseases. The mechanism may involve altered cytokine rhythms or specific pathogen characteristics. Nurses detecting this pattern should document temperature trends carefully, report abnormal findings, and consider possible causes. Other fever patterns include: continuous (persistent with little variation); intermittent (returns to normal between spikes); remittent (fluctuates but doesn't return to normal); and relapsing (periods of fever alternating with afebrile periods). Accurate temperature assessment includes noting pattern, associated symptoms, response to antipyretics, and impact on patient comfort and function.
