A leiomyoma is a benign tumor of smooth muscle that can rarely occur in the scrotum, arising from the dartos muscle (the thin layer of smooth muscle in the scrotal wall) or from the smooth muscle of the spermatic cord. It presents as a slow-growing, firm, painless, mobile subcutaneous nodule. While leiomyomas are common in the uterus, they are rare in the male genitalia. They are entirely benign and treated by simple local excision if they cause discomfort or for cosmetic reasons. It is important to differentiate it from other scrotal masses like lipomas (fatty tumors), sarcomas (malignant soft tissue tumors), or hernias. Diagnosis is usually confirmed after surgical removal and pathological examination, which shows characteristic intersecting bundles of spindle-shaped smooth muscle cells without signs of malignancy.
