The primary nursing intervention for a patient experiencing an acute stroke is rapid assessment using the FAST acronym (Face drooping, Arm weakness, Speech difficulty, Time to call emergency) and immediate activation of the stroke response team to ensure timely intervention. Time is critical for ischemic strokes, as tissue plasminogen activator (tPA) must be administered within 4.5 hours of symptom onset, and endovascular thrombectomy within 6-24 hours depending on criteria. Nursing actions include: monitoring airway, breathing, and circulation; performing neurological assessments using the NIH Stroke Scale; obtaining vital signs; establishing IV access; preparing for CT scan; and administering tPA if ordered after ruling out contraindications. Documentation should include time of symptom onset, neurological findings, and interventions. Patient education focuses on stroke prevention and recognition. Post-stroke care involves rehabilitation and secondary prevention (antiplatelets, statins, blood pressure control). Rapid response improves outcomes and reduces disability.
