Pulmonary hamartoma in respiratory cytology may show fragments of cartilage, adipose tissue, and bronchial epithelial cells, but these are rarely seen in sputum or bronchial washings as hamartomas are well-circumscribed and do not exfoliate readily. When sampled by fine-needle aspiration, they show benign mesenchymal elements (cartilage, fat, smooth muscle) and sometimes epithelial components. In sputum cytology, hamartoma is essentially never diagnosed; it requires imaging and biopsy. The question highlights that some entities are not amenable to cytological diagnosis via exfoliative specimens.
