The evaluation phase determines the extent to which patient goals have been achieved and whether nursing interventions were effective. Steps include: collecting data related to outcomes; comparing data with expected outcomes; analyzing reasons for outcomes; modifying care plan as needed; and documenting evaluation. Evaluation is ongoing, not just at discharge. If goals met: document progress and possibly discontinue related interventions. If goals partially met or not met: reassess situation; consider if outcomes realistic; evaluate implementation; and revise plan. Evaluation requires critical thinking: were assessments accurate? diagnoses appropriate? outcomes measurable? interventions correctly implemented? Factors affecting outcomes: patient adherence, changing condition, new problems, external factors. Documentation should show progress toward goals. Evaluation ensures accountability, promotes evidence-based practice, and improves quality of care. It completes the nursing process cycle, with findings leading back to reassessment. Effective evaluation requires measurable outcomes established in planning phase.
