The diagnostic procedure where a spring-loaded needle is inserted, typically through the rectal wall under ultrasound guidance, to obtain small cylindrical cores of prostate tissue for microscopic examination is called a transrectal ultrasound-guided prostate biopsy (TRUS biopsy). It is the standard method for definitively diagnosing prostate cancer. During the procedure, an ultrasound probe is inserted into the rectum to visualize the prostate in real-time. The biopsy needle is then fired through a guide on the probe, taking 10-12 (or more) tissue samples from systematic areas of the prostate, with a focus on the peripheral zone. These cores are sent to a pathologist who examines them for cancer cells and assigns a Gleason score. While generally safe, risks include bleeding, infection (including rare but serious sepsis), and temporary urinary symptoms.
