The primary nursing concern for a patient with a spinal cord injury (SCI) is respiratory compromise and neurogenic shock due to disruption of autonomic pathways, especially in cervical injuries. High cervical injuries (C1-C4) affect diaphragm function, potentially requiring mechanical ventilation. Neurogenic shock presents as hypotension, bradycardia, and warm dry skin due to loss of sympathetic tone. Nursing interventions include: maintaining airway/breathing; monitoring for shock; immobilizing the spine until cleared; preventing complications (pressure ulcers, DVT, contractures); managing bladder/bowel dysfunction; and watching for autonomic dysreflexia (hypertensive emergency in injuries above T6). Documentation includes neurological level, motor/sensory function, and autonomic stability. Rehabilitation focuses on maximizing independence. Patient education covers long-term management and prevention of complications. Acute management aims to prevent secondary injury and stabilize the patient.
