Spiritual assessment evaluates beliefs, values, and practices that give meaning and purpose to life, identifies spiritual needs, and guides interventions for spiritual well-being as part of holistic care. Components: beliefs about health, illness, suffering, and death; sources of hope and strength; religious practices important to patient; and spiritual concerns or distress. Tools: FICA (Faith, Importance, Community, Address); HOPE (Hope, Organized religion, Personal spirituality, Effects on care). Nurses approach with respect and openness, without imposing own beliefs. Documentation includes patient's expressed spiritual needs and interventions. Spiritual assessment detects: spiritual distress, need for chaplain referral, conflicts between beliefs and treatment. Regular monitoring guides: accommodation of religious practices; support for existential questions; and integration of spiritual resources into care. Nurses educate about available spiritual support. This assessment is crucial during serious illness, end-of-life care, or when patients express spiritual concerns. It respects patient's dignity and supports coping.
