Alcian blue stain in respiratory cytology highlights acidic mucopolysaccharides (sialomucins and sulfomucins) which stain blue. It is used to: 1) Identify goblet cell hyperplasia or metaplasia (blue cytoplasmic mucin), 2) Detect adenocarcinomas with mucin production, 3) Distinguish between neutral (PAS-positive) and acidic mucins, 4) Highlight the capsule of Cryptococcus neoformans (though mucicarmine is better). Alcian blue at pH 2.5 stains sialomucins; at pH 1.0 stains sulfomucins. It is often combined with PAS (AB-PAS) to differentiate mucin types. This stain helps characterize glandular differentiation and certain infectious organisms.
