Pulsus alternans is a pulse pattern where there is a regular alternation of strong and weak beats despite a regular rhythm. This occurs in conditions of severe left ventricular dysfunction, such as advanced heart failure, where the heart muscle is so weakened that it cannot contract with consistent force with each beat. The alternating strong and weak contractions create this distinctive pulse pattern. Detection of pulsus alternans is significant because it indicates serious cardiac impairment and poor prognosis. Nurses need to be skilled in detecting this subtle pulse variation as it requires prompt medical attention and may indicate the need for adjustment of heart failure medications or other interventions.
