Lithiasis (kidney or bladder stones) can cause atypical urothelial cells that mimic carcinoma in urine cytology. The mechanical irritation from stones leads to reactive changes including nuclear enlargement, prominent nucleoli, and increased N/C ratios. However, these reactive cells typically maintain uniform chromatin and smooth nuclear membranes. Other conditions causing similar atypia include instrumentation, intravesical therapy (BCG, chemotherapy), radiation, and viral infections. Distinguishing reactive atypia from true malignancy requires careful evaluation of nuclear features and clinical correlation.
