This statement is true. High-grade prostatic intraepithelial neoplasia (HGPIN) is widely accepted as a direct precursor lesion to invasive prostatic adenocarcinoma. On biopsy, HGPIN shows cells that look malignant (with enlarged nuclei and prominent nucleoli) but are still confined within the basement membrane of the prostatic glands—it is carcinoma in situ. Finding HGPIN on a prostate biopsy indicates a significantly increased risk of finding invasive cancer on subsequent biopsies. Therefore, it often prompts closer surveillance with repeat biopsies. In contrast, low-grade PIN (LGPIN) is not considered clinically significant.
