Strongyloides stercoralis presents a diagnostic challenge because larval output in stool is often low and intermittent. When routine direct smear and formalin-ethyl acetate concentration of stool are negative, specialized techniques with higher sensitivity are required. The Baermann funnel technique is considered one of the most sensitive direct methods. It exploits the larvae's motility and positive thermotaxis. Stool is placed in warm water on gauze in a funnel; live larvae migrate out and settle at the bottom of the stem, where they can be collected and identified. Agar plate culture is another sensitive method. Serology (antibody detection by ELISA) is highly sensitive and is often used when parasitic confirmation is difficult, but it cannot distinguish between past and current infection. The Kato-Katz (A) is for helminth eggs, not larvae. The Scotch tape test (C) is for Enterobius. Routine O&P (D) has low sensitivity for Strongyloides.
