When normal endocervical columnar epithelial cells are viewed from their side profile in a cytological preparation, they characteristically align in a linear, orderly fashion that resembles a 'palisade' or 'picket fence' arrangement. This occurs because the cells are tall and columnar with their nuclei positioned at the basal end of the cell (closest to the basement membrane). When these cells exfoliate and settle on a slide, they often maintain this orientation, appearing as a row of tall, narrow cells with nuclei all at approximately the same level. This distinctive pattern is an important diagnostic feature that helps identify endocervical cells and confirms adequate sampling of the transformation zone. The palisade arrangement contrasts with the three-dimensional balls of endometrial cells and the dispersed, individual arrangement of squamous epithelial cells.
