The standard initial surgical treatment for a confirmed testicular germ cell tumor is a radical inguinal orchiectomy. This procedure involves making an incision in the groin (inguinal region), clamping the spermatic cord before manipulating the testicle, and then removing the entire testis and spermatic cord up to the internal inguinal ring. The inguinal approach is critical—it prevents the potential spread of cancer cells into the scrotum or disruption of lymphatic drainage patterns, which could occur if the scrotum itself were incised. After removal, the tumor is staged pathologically. This surgery serves both as definitive treatment for the primary tumor and provides tissue for accurate histological diagnosis, which guides all further therapy (such as surveillance, chemotherapy, or additional surgery like retroperitoneal lymph node dissection).
