Chronic Lymphocytic Leukemia (CLL) is characterized by a proliferation of mature-appearing but functionally incompetent B-lymphocytes. On a blood film, the classic finding is an increased number of small, mature lymphocytes. A key feature that helps distinguish these from normal lymphocytes is the presence of 'smudge' or 'basket' cells. These are fragile CLL cells that have been damaged during slide preparation, causing their nuclei to appear smeared out. While not diagnostic alone, seeing numerous smudge cells alongside an absolute lymphocytosis is highly suggestive of CLL.
