Transfusion-Associated Graft-versus-Host Disease (TA-GVHD) is a rare but almost universally fatal complication. It occurs when viable donor T-lymphocytes present in a cellular blood product (RBCs, platelets, granulocytes) engraft in an immunocompromised recipient. These donor T-cells recognize the recipient's tissues as foreign and mount an immune attack. Key features include: 1) **Onset**: Typically 1-2 weeks post-transfusion. 2) **Symptoms**: Fever, a characteristic erythematous maculopapular skin rash (often starting on the palms and soles), liver dysfunction (jaundice, elevated enzymes), diarrhea, and pancytopenia (due to bone marrow suppression). 3) **High mortality** (>90%) due to bone marrow aplasia and infection. 4) **Prevention**: Irradiation of cellular blood products to inactivate donor lymphocytes. TA-GVHD is distinct from solid organ transplant GVHD and is not associated with allergic symptoms or immediate hemolysis.
