The congenital ventral curvature (downward bending) of the penis that is frequently associated with hypospadias is called chordee. In hypospadias, the urethral meatus is on the ventral side of the penis. The tissue on the ventral side (the urethral plate) is often underdeveloped and fibrotic, acting like a tight bowstring that pulls the penis into a curved shape, especially during erection. This curvature can be significant and, if not corrected, can interfere with sexual function. Surgical repair of hypospadias typically involves not only creating a new urethral opening at the tip of the glans but also releasing this chordee to straighten the penis. Chordee can also occur without hypospadias (congenital chordee without hypospadias).
