A key histological hallmark of prostate adenocarcinoma is the loss of the basal cell layer in the malignant glands. In normal prostate tissue and benign conditions like BPH, glands are lined by two layers of cells: an inner layer of secretory columnar cells and an outer layer of basal cells. In adenocarcinoma, this basal cell layer is absent. This feature is crucial for diagnosis and can be highlighted with immunohistochemical stains for basal cell markers (like p63 and high molecular weight cytokeratin). Other features include nuclear atypia (enlarged nuclei with prominent nucleoli), infiltrative growth pattern, and perineural invasion. The presence of a double cell layer would be characteristic of benign tissue.
