Leydig cell tumors, which arise from the testosterone-producing interstitial cells, can secrete hormones. While they most commonly produce androgens, some produce estrogens. In adult men, excess estrogen can lead to gynecomastia, which is the benign enlargement of breast tissue. This is considered a paraneoplastic syndrome—a symptom or sign that is the consequence of a hormone or other substance produced by the tumor. Other possible findings include loss of libido and testicular atrophy (due to feedback inhibition of pituitary gonadotropins by estrogen). In prepubertal boys, a Leydig cell tumor causing androgen excess would lead to precocious puberty (early virilization). Therefore, the presence of gynecomastia in a man with a testicular mass should raise suspicion for a sex cord-stromal tumor like a Leydig cell tumor.
