Cognitive assessment evaluates mental processes including orientation, memory, attention, language, and executive function to identify impairments, monitor changes, and guide care planning. Components: orientation (person, place, time, situation); memory (immediate, recent, remote); attention and concentration (serial sevens, spelling backwards); language (naming, repetition, comprehension); and executive function (abstraction, judgment, problem-solving). Standardized tools: Mini-Mental State Examination (MMSE), Montreal Cognitive Assessment (MoCA), Clock Drawing Test. Nurses also assess: functional impact on activities of daily living; and behavioral changes. Documentation includes specific test results and observations. Cognitive assessment detects: delirium (acute, fluctuating), dementia (chronic, progressive), depression (pseudo-dementia). Regular monitoring guides: safety interventions; communication strategies; and referral to neurology/psychiatry. Nurses educate families about cognitive changes and management. This assessment is crucial for elderly, neurological patients, or those with medication affecting cognition. Early detection allows intervention.
