A hypophysectomy is the surgical procedure for the partial or complete removal of the pituitary gland. It is most commonly performed to treat pituitary tumors (adenomas) that are causing hormone overproduction (as in acromegaly, Cushing's disease, or prolactinomas resistant to medication) or that are compressing nearby structures like the optic chiasm, causing vision loss. The surgery can be approached transsphenoidally (through the nose and sphenoid sinus, which is most common and minimally invasive) or, less often, transcranially (through an opening in the skull). Because the pituitary is the 'master gland,' its removal necessitates lifelong hormone replacement therapy for multiple hormones, including cortisol (hydrocortisone), thyroid hormone (levothyroxine), sex hormones, growth hormone, and sometimes vasopressin (for diabetes insipidus). Careful endocrine management is crucial before and after surgery.
