High fever and chills are not typical symptoms of chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS). These are hallmark signs of an acute bacterial infection, such as acute bacterial prostatitis. CP/CPPS, the most common form of prostatitis, is characterized by pelvic pain (in the perineum, lower abdomen, testicles, or penis) and urinary symptoms (frequency, urgency, dysuria) lasting for at least 3 months, in the absence of a detectable bacterial infection. Symptoms can wax and wane and are often associated with significant negative impact on quality of life. Sexual dysfunction (like painful ejaculation or erectile difficulties) is common. The absence of systemic signs like fever helps differentiate it from acute infectious prostatitis.
