A key diagnostic maneuver in differentiating a hydrocele from a hematocele is transillumination. A hydrocele, which contains clear serous fluid, will typically transilluminate. This means that when a bright light (like from a penlight) is placed against the swollen scrotum in a dark room, the light will pass through the fluid-filled sac, causing the entire scrotum to glow with a red or orange hue. This is a classic sign of a hydrocele. In contrast, a hematocele contains blood, which does not allow light to pass through. Therefore, a hematocele will not transilluminate; the light is blocked, and the scrotum remains dark. While both may present as scrotal swellings, the history (acute trauma suggests hematocele) and this simple bedside test are very helpful in distinguishing them.
