Antipsychotics typically cause hyperprolactinemia due to D2 receptor blockade in the tuberoinfundibular pathway, leading to symptoms like amenorrhea (not menorrhoea) and decreased libido in women, and gynecomastia and decreased libido in men. They do NOT cause menorrhoea (heavy menstrual bleeding) or increased libido. Low-potency antipsychotics often cause orthostatic hypotension due to alpha-1 adrenergic blockade.
