Responses: assess why question is asked (curiosity, testing boundaries, seeking connection); redirect to therapeutic focus ('I wonder if you're asking because...'); set limits politely ('I prefer to keep our focus on your care'); or use minimal self-disclosure only if therapeutic and brief. Boundary violations occur when nurse shares excessively or becomes friend rather than professional. Documentation of significant boundary challenges may be needed.
