In a complement fixation test, patient serum (potential antibody) is mixed with antigen and a known amount of complement. If the antibody is present, it binds antigen and 'fixes' (uses up) the complement. Sensitized sheep red blood cells (SRBCs) are then added. If complement was fixed (positive test), no lysis occurs and the SRBCs settle. If no antibody was present (negative test), complement remains free and lyses the SRBCs.
