Penile Intraepithelial Neoplasia (PeIN) is considered a premalignant lesion of the penis. It represents squamous cell carcinoma in situ, meaning there are dysplastic (abnormal) cells confined to the epithelium (the top layer of skin) that have not yet invaded into the deeper tissue. If left untreated, PeIN has the potential to progress to invasive squamous cell carcinoma. It is strongly associated with infection by high-risk strains of Human Papillomavirus (HPV), particularly types 16 and 18. Clinically, it can appear as a velvety red plaque (Erythroplasia of Queyrat) or a scaly plaque (Bowen's disease). Treatment involves local destruction or excision to prevent malignant progression. Therefore, it is not benign, not caused by bacteria, and is a direct precursor to cancer.
