The Dutch guidelines presented emphasize a critical safety rule: the ABO blood group of a patient must be determined on at least two separate blood samples, and there must be no discrepancies between the results. If the two samples yield different ABO types, this is a major red flag indicating a serious error. In such a case, the transfusion laboratory must NOT issue any blood for that patient until the discrepancy is fully resolved. The cause must be investigated immediately.
