In older men (typically over 50), acute bacterial prostatitis is most commonly caused by gram-negative enteric bacteria, with Escherichia coli (E. coli) being the predominant pathogen. These bacteria are part of the normal intestinal flora and can cause urinary tract infections that ascend to the prostate. This is in contrast to younger, sexually active men, where sexually transmitted organisms like Chlamydia trachomatis and Neisseria gonorrhoeae are more common. Older men are more susceptible due to factors like benign prostatic hyperplasia (BPH), which can cause urinary retention and stasis, and instrumentation of the urinary tract (e.g., catheterization). Treatment requires antibiotics that penetrate the prostate well, such as fluoroquinolones (e.g., ciprofloxacin) or trimethoprim-sulfamethoxazole.
