Platelet concentrates contain a significant amount of donor plasma. A platelet unit from a type O donor contains anti-A and anti-B antibodies in the plasma. When transfused to a type A recipient, the donor's anti-A antibodies in the plasma can bind to the recipient's type A red blood cells, potentially causing hemolysis. This is why platelets are ideally ABO-matched to the recipient. However, because the volume of plasma in a platelet unit is relatively small (about 200-300 mL), the hemolytic reaction is usually mild or subclinical, especially if the donor's antibody titer is low. To mitigate this risk, some blood centers produce platelets suspended in additive solutions or reduce the plasma volume. For recipients who are small, have high antibody titers in the donor unit, or receive many units, ABO matching or screening donor plasma for high-titer anti-A/B is recommended.
