This statement is generally true and reflects an important quality assurance principle in cervical cytology. The transformation zone (TZ) is the area between the original squamocolumnar junction and the current squamocolumnar junction where squamous metaplasia occurs. This is the most important area to sample because virtually all cervical precancers and cancers arise here. Since the transformation zone contains both squamous epithelium (ectocervix) and glandular epithelium (endocervix), the presence of endocervical cells (and/or metaplastic squamous cells) in a Pap smear suggests that the sampling device reached and sampled the transformation zone. Smears lacking endocervical/transformation zone components are considered 'less than optimal' or 'unsatisfactory' for evaluation because they may have missed the critical area where abnormalities are most likely to occur. However, it's important to note that in postmenopausal women or after certain procedures, the transformation zone may recede into the endocervical canal, making it harder to sample, and absence of endocervical cells doesn't always indicate inadequate sampling.
