The electronic (or computer) crossmatch is a method to confirm blood compatibility without performing a serological crossmatch. It relies on stringent computer checks. Essential steps/criteria include: 1) The patient must have two historical ABO and Rh(D) typings on record that agree. 2) The patient must have a current negative antibody screen (within the last 72 hours typically). 3) The patient must have no history of clinically significant antibodies. 4) The computer system must compare the donor unit's ABO/Rh information with the patient's confirmed type and ensure they are compatible. 5) The system must have logic to prevent the release of ABO-incompatible blood. The electronic crossmatch does NOT involve physically mixing patient serum and donor cells. It is faster and reduces human error but is only applicable to patients meeting strict safety criteria.
