The string test (Entero-Test) is a non-invasive procedure used to obtain duodenal contents for examination. A patient swallows a gelatin capsule on a string; the capsule dissolves in the stomach, and the weighted string passes into the duodenum. After several hours, the string is retrieved. Bile-stained mucus adherent to the string represents duodenal secretions. Microscopic examination of this material can reveal trophozoites of Giardia lamblia or, less commonly, Strongyloides stercoralis larvae. A positive test, finding these organisms, directly implicates the duodenum and upper small intestine as the site of pathology. This is particularly significant for diagnosing giardiasis when stool examinations are repeatedly negative due to intermittent cyst shedding or low parasite burden. It provides parasitological confirmation without the need for invasive endoscopy.
