Reactive mesothelial cells in pleural fluid cytology typically show moderate cellular enlargement, increased nuclear/cytoplasmic ratio, prominent but regular nucleoli, and occasional multinucleation. However, they maintain a uniform appearance with smooth nuclear membranes and evenly distributed chromatin. They may form small clusters with 'windows' (clear spaces) between cells due to long microvilli. Mitotic figures can be present but are typical. Reactive changes occur in response to inflammation, infection, or irritation. Distinguishing reactive mesothelial cells from adenocarcinoma cells requires careful evaluation of nuclear features and may require immunocytochemistry.
