Finding eosinophils in sputum cytology indicates allergic or parasitic inflammation. Eosinophils are granulocytes with bilobed nuclei and coarse, bright orange-red cytoplasmic granules on Romanowsky stains. Their presence suggests conditions such as asthma, allergic bronchopulmonary aspergillosis, eosinophilic pneumonia, or parasitic infections (e.g., Strongyloides, Paragonimus). Charcot-Leyden crystals, derived from eosinophil granules, may also be present. The quantity of eosinophils can be semi-quantified and correlated with clinical symptoms to guide diagnosis and monitor response to treatment in allergic pulmonary diseases.
