Genitourinary assessment evaluates urinary and reproductive system function to identify problems, monitor conditions, and guide interventions for elimination and reproductive health. Components: urinary output (amount, frequency, characteristics, dysuria); fluid intake; bladder distention; costovertebral angle tenderness; and for relevant patients, genital examination. Nurses also assess: continence; urinary retention; symptoms of infection (frequency, urgency, pain); and reproductive concerns (menstrual history, sexual function, discharge). Documentation includes specific findings and patient reports. Special assessments: post-void residual measurement; urinalysis interpretation. GU assessment detects: urinary tract infections, renal impairment, prostate enlargement, incontinence. Regular monitoring guides: fluid management; catheter care; medication administration; and referral to urology. Nurses educate about prevention of UTIs, Kegel exercises for incontinence, and safe sexual practices. This assessment is crucial with renal disease, urinary symptoms, pregnancy, or medications affecting urinary system.
