The primary nursing intervention for epilepsy is seizure precautions and management during seizures while promoting medication adherence and lifestyle modifications to control seizures. Nursing actions during seizure: ensure safety (prevent injury, protect head, turn to side); time seizure; observe characteristics; stay with patient; after seizure, maintain side-lying position, reorient, check breathing. Between seizures: administer antiepileptic drugs as prescribed; monitor for side effects (sedation, rash, liver toxicity); educate about triggers (sleep deprivation, alcohol, stress, flashing lights); and promote safety (shower not bath, no swimming alone, caution with driving). Documentation includes seizure description, duration, and postictal state. Patient education about medication adherence (sudden withdrawal can cause status epilepticus), seizure diary, and first aid. Nurses coordinate with neurology for medication adjustments. Refractory epilepsy may require surgical evaluation. Quality of life issues include driving restrictions and stigma. Support groups help coping.
