Finding 'tadpole cells' in sputum cytology is significant as they are characteristic of well-differentiated squamous cell carcinoma. Tadpole cells are elongated malignant squamous cells with a bulbous head containing the nucleus and a slender, tapering cytoplasmic tail. They represent abnormal keratinization and cellular dysplasia. Their presence, along with other keratinizing malignant cells like 'fiber cells' and anucleate squamous cells, supports a diagnosis of squamous cell carcinoma. While not pathognomonic (they can rarely be seen in benign conditions), in the right context they strongly suggest malignancy.
