Wound care and management focuses on promoting healing, preventing infection, and maintaining tissue integrity through appropriate dressing selection, infection control, and addressing underlying causes. Wound assessment includes: location, size, depth, tissue type (granulation, slough, eschar), exudate, odor, surrounding skin, and pain. Principles: maintain moist environment (not wet); remove necrotic tissue; control exudate; protect surrounding skin; and minimize pain. Dressing selection depends on wound characteristics. Nurses also address factors affecting healing: nutrition (protein, vitamin C, zinc); oxygenation; diabetes control; and pressure relief. Documentation with photographs (with consent) tracks progress. Infection signs: increased pain, erythema, edema, purulent exudate, fever. Complex wounds require specialist referral. Patient education on self-care and prevention of recurrence.
