When a suspected acute hemolytic transfusion reaction occurs (symptoms like fever, chills, pain, hypotension during transfusion), the immediate priority is to stop the transfusion to prevent further infusion of the incompatible blood. This is the single most important first action. The intravenous line should be kept open with normal saline (using a new IV bag and tubing) to maintain vascular access for administering emergency medications and fluids. The blood unit and the attached administration set should then be disconnected and sent back to the blood bank along with a new blood sample from the patient (from a different site) for investigation. While notifying the doctor and checking vital signs are also critical immediate steps, stopping the infusion is the action that directly addresses the ongoing cause of the reaction.
