Persistent or recurrent symptoms after treatment for trichomoniasis are common. The most critical step is to ensure all sexual partners from the relevant period (typically the last 60 days) are identified, tested, and treated. Men are frequently asymptomatic carriers. If her partner(s) were not treated, she will almost certainly be reinfected immediately after her own treatment—a phenomenon known as the 'ping-pong' effect. Simply retreating her without addressing the source of reinfection is futile and may contribute to drug resistance. While drug resistance (B) is a possibility and should be considered if partner management is confirmed, it is less common than reinfection. A test-of-cure (C) is positive by definition here (wet mount is positive). Switching to a different regimen without partner therapy (D) will likely also fail due to reinfection. Therefore, comprehensive partner management is the cornerstone of effective STI control and must be the first and most important intervention.
