Ibuprofen, an NSAID, inhibits cyclooxygenase (COX), reducing prostaglandin synthesis. In the kidney, prostaglandins (especially PGE2 and PGI2) help maintain renal blood flow and glomerular filtration rate (GFR) by dilating afferent arterioles. In a patient with heart failure (reduced effective circulating volume), NSAIDs can cause significant vasoconstriction of renal arterioles, leading to a drop in GFR and oliguria.
